I, {name-1-first-name} {name-1-last-name} hereby make application for membership to Plumsteadville Volunteer Fire Company. I understand that my membership acceptance or rejection is by vote of the company members and that my membership, if granted, may be revoked at any time in accordance with the company’s Constitution and Bylaws. 

I authorize Plumsteadville Volunteer Fire Company’s Membership Committee and Executive Council to investigate all information pertaining to my application. I understand that this will include, but not be limited to, a criminal history check and driver's license check with any appropriate authority. 

I understand that if I am accepted, I will be on probation for a minimum of twelve months from the date of acceptance. 

Any misinformation contained in this application shall be grounds for immediate rejection. Disciplinary action up to and including membership termination may be instituted for current members. 

All sections of this application must be completed in full. If a question does not apply to you, please enter N/A. Incomplete applications will not be accepted or processed.

Dues are $5.00, payable in advance.
